PRESCRIPTION REFILLS

Dear NGMH Patient

We would like to inform you that all telehealth appointments (appointments over the phone with the Doctor, including Prescription refills/renewals,) will be charged to your insurance, or directly to you if you are a self-payer. Co-pays will apply. All appointments with Dr. Bernstein is required and mandated to be documented as per regulations. 

All prescription renewals are subject to a Telehealth and/or an Office Visit with the doctor. You will be notified first.
Specifically, if your last visit was more than 3 months.

In the meantime we ask our patients to go to our website for renewal of your prescriptions under the Patient Portal. Someone from NGMH will confirm receipt of your request. We have provided the link for your convenience below. Due to the volume of patients, we apologies we are limited to accept phone calls. We will do our best for those who need accommodating to continue with your patient care.
